Write on the CD / DVD the title of the video, your full name, address, phone number, and email address. Print / Sign the authorization form and mail it with the video. Entrants must submit a short video between 40 and 60 seconds in length based on the book “Two Cents per Mile: Will President Obama Make it Happen With the Stroke of a Pen?” by Nevres Cefo.
